Delaware Family Voices Chronic Conditions Survey – Your Input Needed

Do you live in Delaware and have a child with one of the following chronic medical conditions?

  • Diabetes, any type
  • Chronic respiratory conditions
  • Chronic cardiac conditions

If so, Delaware Family Voices needs your input. We would greatly appreciate you taking the time to complete the Delaware Family Voices Chronic Conditions Survey and help us learn more about what you need to better manage your child’s condition.

Survey Specifics:

The survey’s objective is to assist Delaware Family Voices to identify the unmet needs of Delaware families with children who have chronic cardiac or respiratory conditions, or diabetes.  The survey outcome will be a roadmap of currently available services, what services might be lacking, and how we can help families find and access the services they need.

Additionally, we will make recommendations to address these unmet needs to service providers, policy makers, and others, as well as advocate for and monitor the implementation of these same recommendations.

Lastly, though our survey is directed towards three specific chronic conditions, our objective is to represent all children with any chronic health condition. Any recommendations we make will also have the intentions of assisting any child with a chronic health condition.

    When I originally commented I clicked the “Notify me when new comments are added” checkbox and now each
    time a comment is added I get four e-mails with the same comment.
    Is there any way you can remove me from that service?
